SSVV Personal Safety Logbook

The Management or the Authorized Representative of the company wishing to order copies of the Personal Safety Logbook hereby certifies that he/she shall comply with the conditions attached to the issue of the SSVV Personal Safety Logbook by the Foundation Cooperation for Safety (SSVV - Stichting Samenwerken Voor Veiligheid), of Leidschendam, the Netherlands, whereby he/she:

 

. shall use the Personal Safety Logbook in   accordance with the instructions included in
  the Logbook,
. shall ensure that, if so requested, a   representative of the Personal Safety Logbook
  Control Committee will be provided with the   opportunity to verify the validity of the
  information contained in the associated files,

. shall ensure that in the event of misuse the Personal Safety   Logbook shall, if so  requested, be returned to the SSVV,

. is aware that demonstrable fraud can result in the SSVV's disclosure   of the name of the company concerned.

  Acceptance of the conditions of this Declaration is equivalent to   signing a contract!
